AUTHORIZATION TO USE PHOTOS/VIDEOS

I hereby grant Subies of the Corn the absolute and irrevocable right and unrestricted permission to use any photograph, video or audio that either of them or their agent has taken or may take of me on in which I may be included with others. Such use shall include the right and authorization to reuse, edit, alter, exhibit, publish and/or distribute any such photo, video or audio in any publication or media, including website entries, as well as the use of my name in any copy accompanying a photograph or video. In addition, I waive (a) the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears, and (b) any right to royalties or any other compensation for the use of a photograph, video, audio or any copy (including my name) related thereto. I hereby hold harmless and release and forever discharge Subies of the Corn from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other person’s action on my behalf or on behalf of my estate have or may have by reason of this authorization. I am 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.

If the person signing is under the age of 18, there much be a consent by a parent or guardian, as follows: I hereby certify that I am the parent of guardian of the named above, and do hereby give my consent without reservation to the foregoing on behalf of this person.
